A gallbladder removal (= cholecystectomy) refers to the removal of the entire gallbladder, usually containing gallstones that have caused pain
(if the gall bladder is inflamed ( cholecystitis) .
The gall bladder is the storage organ for bile (approx. 700 ml daily) which is produced by the liver. Bile acids (in the bile) promote the digestion of fats and the absorption of fat-soluble vitamins.

Why is nutritional therapy important?

After the operation, nutritional therapy supports you in integrating a diet that you can easily tolerate into your everyday life. A bile diet as it was often practiced in the past no longer exists. Digestive problems can arise shortly after a gallbladder removal.

The aim is to supply all of the fatty acids, micronutrients (vitamins and minerals) that are important to you, energy and protein in the measure that your body is optimally supplied with. For this purpose, foods that promote inflammation are specifically avoided and plenty of anti-inflammatory foods are consumed. In the case of overweight / obesity, weight reduction is recommended, which reduces the risk of gallstones and also the risk of inflammation. An anti-inflammatory diet can improve your health.

Aim: A wholesome food that is easily tolerable for you

Possible consequences:

  1. Inflammation of the pancreas (acute pancreatitis)

  2. Injury to the gallbladder wall with leakage of bile into the abdomen and resulting peritonitis

  3. increased risk of gallbladder and bile duct cancer

  4. Postcholecystectomy syndrome (English: postcholecystectomy syndrome

Postcholecystectomy Syndrome:

Normally, there should be no long-term symptoms after a gallbladder removal.

Persistent complaints could have the following causes:

  • Gallstones in the biliary tract, e.g. in the common bile duct

  • Biliary strictures

  • Stenosis of the major duodenal papilla

  • Other diseases e.g.

    • Gastroduodenal ulcer disease

    • Pancreatitis

    • Irritable bowel syndrome


After the cholecystectomy, some patients develop new symptoms despite the absence of initial symptoms. Meals rich in fat are no longer so well tolerated, and malabsorption of fats may occur (lack of intermittent bile secretion from the gall bladder). These complaints can then also be treated with nutritional therapy.

Overall, of course, other possible causes should be excluded diagnostically, ie before / also during the cholecytectomy, a sonographic assessment should be carried out with, if necessary, revision of the biliary tract.

Nutritional therapy

  • Get enough fiber (> 30 g / day) through natural foods, possibly fiber substitution

  • Sufficient fluid: total fluid per day: 30 - 35 ml per kg body weight and day. Attention, this is not just a drinking liquid. The liquid from food (approx. 500 - 800 ml) must still be subtracted from this, so that an amount of approx. 1.3 - 2 l of drinking liquid is created.
